At Home Services

At Home Services provide individuals with an alternative to traditional inpatient supports. This particular model of service delivery ensures the client is able to access individualized care designed to assist them in maximizing and maintaining independence. For some, this may mean a few hours a week of support, for others, there may be a need for up to 24 hours of daily supports. Either way, At Home Services provides the client with flexible options for managing their personal care needs.

At Home Services can occur through short term transitional supports which may be required following discharge from an acute care setting, or, can be a long term service method if there is a need for a constant continuum of care. This model of service delivery affords the client the opportunity of implementing flexible care options which can be revisited as the needs of the client change. For some, this can mean a decrease in services, for others, an increase in services.

For all of us, maintaining our independence is fundamental to how we live out our lives. Therefore, maintaining independence is paramount. Depending on the issues at play, Life Skills Counsellors (the name for staff who provide the service) can help the client maintain independence in a multitude of areas. For example, Life Skills Counsellors can help the client acquire new skills, or, re-acquire previously known skills. They can assist with reintegration, whether back to the family home, into the community, or even within the school or workplace setting. Functional independence can be maintained through supports with ADL's, home management tasks and budget. Other supports can include accompanied medical visits so as to ensure an accurate information exchange occurs. For some, assistance in all of the above areas may be a necessity, for others, the needs may be isolated to one particular area.

At Home Services are clinically supervised supports. Direct client services are provided to the client by trained Life Skills Counsellors. Our Life Skills Counsellors have an expertise in working with a variety of disability groups and have a particular expertise in working with individuals who have sustained an acquired brain injury. Life Skills Counsellors are clinically supervised by a multi-disciplinary team of therapist so as to ensure goal oriented service delivery.

The process of applying for At Home Services is simple. Referrals can be self initiated, medically driven, or can be received directly from a funding source, such as WSIB, Blue Cross, Department of Veteran's Affairs Canada or a Third Party Insurer. Once a referral has been made, the Admissions Coordinator contacts the client to make an appointment so as to complete the initial Pre-Admission Assessment interview. This process provides us with the opportunity of gathering the information we need to ensure we can provide the client with the right level of support. The Admission Coordinator is responsible for securing funding for services from the payer source and keeps the client informed as to where things are in process.

Once funding has been approved, the Admission Coordinator then works with other members of the Phoenix Team to bring together a team of Life Skills Counsellors who would be best suited to work with the client.

Under the umbrella of At Home Services, the client can access direct therapies if and when required, and as approved by the funding source. Clinical services can also be provided in the home and include Cognitive Communication Therapy, Functional and Cognitive Occupational Therapy and Individual or Family Counselling. The Phoenix Network can also coordinate additional services such as in house Physiotherapy.
